Eligibility Evidence

PROOF OF ADDRESS

The photo/document must clearly show the applicant's name and address (printed not handwritten) and the type of document it is.

-- If using a Driving Licence as evidence it must be a current valid UK photocard licence. (Driving Licence can be used as proof of address or proof of age, but not both.)

-- If using a utility bill or bank or credit card statement or a DWP letter as evidence then the uploaded copy must show its issue date which must be within the last 3 months.

-- If using a Council Tax bill then it must clearly indicate it is for the current year.

-- If the applicant is Under 21 or has a legal appointee and is not able to supply any of the above documents then an alternative letter may be accepted as proof of address but it must be a properly printed letter and of an official nature.

PROOF OF AGE

The photo/document must clearly show the applicant's name and the type of document it is.

-- If using a Birth Certificate as evidence then your name must not have changed. If changed by marriage then a photo/scan of your Marriage Certificate will also be required.

-- If using a Driving Licence as evidence it must be a current valid UK photocard licence. (Driving Licence can be used as proof of address or proof of age, but not both.)

-- If using your Passport it must be a copy of the personal details page.

-- Finally a letter confirming entitlement to state pension can be used.

PROOF OF DISABILITY

There are seven categories of disability in the context of entitlement for a bus pass and documentary proof is required as follows:

CATEGORY A: Blind or Partially Sighted

-- A certificate of visual impairment

-- A signed letter/report from an ophthalmologist confirming you are severely sight impaired (blind) or sight impaired (partially sighted).

-- A 'Category A medical evidence form' (see council website) completed by a suitable professional.

CATEGORY B: Profoundly or Severely Deaf

-- An audiological report from an aural specialist indicating that hearing loss has reached 70-95 dBHL.

-- A 'Category B medical evidence form' (see council website) completed by a suitable professional.

CATEGORY C: Without Speech

-- Letter from your GP, Consultant or Senior Practice Nurse confirming that you are without speech.

-- Personal Independence Payment with a score of at least 8 points in the communicating verbally activity (please provide full award letter confirming points).

-- A 'Category C medical evidence form' (see council website) completed by a suitable professional.

CATEGORY D: Mobility (walking) substantially impaired

-- Higher Rate Mobility Component of Disability Living Allowance.

-- Personal Independence Payment at 8 points or more in the moving around activity (please provide full award letter confirming points).

-- War Pensioners Mobility Supplement or SPVA Notice of Walking Disability.

-- Disabled Person's Parking Badge (Blue Badge) which is valid for at least six months.

-- A 'Category D medical evidence form' (see council website) completed by a suitable professional. You may still be referred for a mobility assessment.

CATEGORY E: Loss of or loss of use of both arms

-- Letter from your GP, Consultant or physiotherapist confirming your condition.

-- A 'Category E medical evidence form' (see council website) completed by a suitable professional.

CATEGORY F: Learning Disability

-- Medical information which demonstrates you have a formal learning disability diagnosis with the code F70, F71, F72 or F73.

-- An Education and Health Care Plan.

-- An Adult Social Care 'Care and Support Plan'.

-- A 'Category F medical evidence form' (see council website) completed by a suitable professional.

CATEGORY G: Unable to Drive on medical grounds

-- Letter from DVLA confirming the refusal/revocation of a driving licence for a minimum period of 12 months.

-- Personal Independence Payment at 10 points or more in the planning and following a journey activity (please provide full award letter confirming points).

-- A 'Category G medical evidence form' (see council website) completed by a suitable professional.

Important - This category does NOT include those who are excluded from holding a licence due to persistent misuse of drugs and/or alcohol.
